This document is designed to be used for the reporting and investigation of incidents.


Incidents should be reported and investigated as soon as practical after the event. The aim of an incident investigation is to identify the contributing factors which led to the incident and to reduce the likelihood of the incident recurring. For example, did the incident occur as a result of poor maintenance, safe work procedures not being followed, lack of training and supervision, etc?
