The Performance Appraisal Form is used to conduct the performance appraisal process (also referred to as a performance review). This process is a means of informing your employees of the quality of their work and identifying areas of their performance which may require improvement.

The process should be used as a positive and constructive tool of measuring your employee’s performance, while also assisting supervisors to provide valuable feedback to employees concerning their job performance.

The performance appraisal process and the form are important tools to assist managers and supervisors in making employment decisions, including, but not limited to, training needs, merit pay adjustments, transfers, promotions, and career development.

Before using this form you should consider, if relevant, your existing human resource policies and practices relating to performance appraisal. The form provided is basically a sample document. It will need to be amended to meet the specific needs of your business.
