Who can use the Hazard Report Form?
The Hazard Report Form can be used by all employers throughout Australia. It can be completed by any worker engaged by the employer, including contractors and temporary workers.
Commentary
The Hazard Report Form (Form) can be used in the workplace to report potential and/or existing hazards that could cause harm to employees, contractors and guests. It typically includes details about the nature of the hazard, its location, and any immediate actions to be taken in order to mitigate the risk.
Examples of hazards include unsafe equipment, poor working conditions, or environmental risks, and is meant to help management address and rectify the issue.
The report should be submitted as soon as a hazard is identified, or when an employee/worker identifies a concern which could lead to injury or illness.
Using the Form helps to maintain a safe working environment and comply with workplace health and safety regulations.
